Transcripts

  • Students (current and former): Transcripts are processed electronically through the Parchment website. To get started, click on the Order Transcripts here link below. First time users will need to register/set up an account with Parchment before an order can be placed. Transcripts will be sent electronically to the destination
    requested within 2 business days.
  • Schools needing an unofficial transcript for enrollment purposes, may fax a request to (801)256-5364. Please note that official transcripts will be mailed via USPS.
  • Pricing: Free for current students. (Parchment will show a cost of $4.40 but is subsidized and will show $0.00 owing). Former students will be charged $4.40 through Parchment. All requests asking to be mailed will be charged an additional $2.75.

Report Cards / Grades

  • Grades and copies of past report cards are available through Skyward:

Family Access

  • Once logged into Skyward, select Report Cards from the menu on the left-hand side of the screen.  From here, you can view and print report cards.

Vaccinations 

  • To enroll at Copper Hills High School, the following vaccinations are needed:
 
Students born between July 1, 1993 and July 1, 1996 –
  • 5 DTP/DTaP/DT – 4 doses if 4th dose was given on/after 4th birthday.
  •   4 Polio* – 3 doses if 3rd dose was given on/after 4th birthday.
  •   2 Measles
  • 2 Mumps
  •  2 Rubella
  •  3 Hepatitis B

 

Students born AFTER July 1, 1996
  •  5 DTP/DTaP/DT – 4 doses if 4th dose was given on/after 4th birthday.
  •  4 Polio* – 3 doses if 3rd dose was given on/after 4th birthday.
  •  2 Measles
  •  2 Mumps
  •   2 Rubella
  •  3 Hepatitis B
  •  1 Varicella (chickenpox) – OR – History of disease is acceptable, parent must sign verification statement
  • 2 Hepatitis A
*Polio – The final dose of polio vaccine administered ON or AFTER August 7, 2009 must be given at the minimum age of 4 years AND a minimum interval of 6 months following the previous dose.  The final dose of polio administered PRIOR to August 7, 2009 will fall under the previous recommendation with a minimum interval of 4 weeks between doses.
